By submitting this form, I understand that this information will be stored securely and my data will be used to process my case and for no other purpose. This form should be used to make a Formal Complaint about the delivery and quality of service received, about the delivery of teaching / supervisory provision or any other matters relating to a programme of study.

Before completing this form, you should have contacted the relevant Department for an Early Resolution and received a written response.

Please ensure that you complete all sections of the form, particularly where you are asked for further explanation or detail. If you do not do this, your complaint form may not be processed. If this is the case you will be contacted by the student case work team and you may need to begin the process again. 

The Head of School / Department may delegate to an appropriate named individual.

In relation to your Early Resolution, which School(s) or Department(s) does the issue relate to?

You should submit your complaint within three months of the issue you wish to complain about, and within 14 days of the date of the School or Department's written response to your Early Resolution.

Is this a group complaint?

Where it is clear that the issues raised in the complaint have affected a number of students, the University will normally allow a group complaint to be submitted. In order to effectively manage the progress of the case, the group will be expected to nominate one student to act as a representative for the group. The University will normally deal with the nominated representative, and the nominated representative will be expected to liaise with the other members of the group.
